Informed Consent for Telehealth Counseling

Draft for review: this document must be finalized by Tina Bilyeu, LMHC before the practice accepts clients.

This document describes what you can expect from counseling with Tina Bilyeu, LMHC (Florida License MH27127), how telehealth sessions work, and your rights as a client. Please read it carefully; by checking the agreement box and typing your legal name during booking, you are signing this consent electronically.

The counseling process

Counseling offers a supportive, confidential space to work toward your goals. Progress depends on many factors and no specific outcome can be guaranteed. You may end therapy at any time.

Confidentiality and its limits

What you share in session is confidential, with exceptions required by law: risk of serious harm to yourself or others, suspected abuse or neglect of a child, elder, or vulnerable adult, or a court order.

Telehealth

Sessions take place by secure video. You are responsible for a private location and a reliable connection on your end. If a session drops, we will attempt to reconnect or reschedule the remaining time.

Emergencies

Telehealth counseling is not an emergency service. If you are in crisis, call or text 988 (in the US), contact your local emergency number, or go to the nearest emergency room.

Fees and cancellation

Session fees are shown at booking and paid in full to confirm the appointment. Cancellations with less than 24 hours notice may be charged. Contact Tina to reschedule.
